Medical/RX Plan Program
AAPI’s Chamber Sponsored Plan is designed for small to mid-sized groups: 5/EE>100/EE
- No required minimum percentage participation
(Example: 50 eligible employees 15 needing coverage in the Medical Plan) - Level-Funded, partially self-insured, model operating much like a traditional fully insured Plan
- Medical/RX Plan offers option for return of surplus claims funding back to the employer group, if any
- Employer never pays more than pre-determined monthly cost equal to 1/12th of annual Plan Maximum, using fixed monthly “premium”
- Monthly billed Plan rates include all fixed required costs + claims funding for medical/RX expenses
- Plan can be cancelled, at Plan year end, with ongoing covering of eligible claims incurred in Plan year
- 12 Plan Options to choose from. Employer can offer employees more than 1 option, if size allows
- $0 copay, 24/7 telemedicine included with Teledoc
- High performance national traditional PPO network options
- Reference Based Pricing (RBP) for Out of Network or AS the “network only” Plan model. Potential claims saving under RBP can deliver 20>30% over traditional PPO
- PPO “Plus” Model > using approved PPO with single case agreement for Plan & Member savings
Life, Dental, Vision, Accident, Critical Illness, Disability Packages
Silver Package: (Averages $30 PEPM)
- Life – $10,000 basic group term life insurance. Coverage reduces 50% at age 70 and is convertible
- Vision – Eye Med network. $10 deductible exam/$25 materials. $130 frames / $130 contacts
- Accident Coverage – Pays scheduled benefits for accidents from burns, dislocations, and broken bones
Gold Package: (Averages $70 PEPM)
- Life – $10,000 basic group term life insurance. Coverage reduces 50% at age 70 and is convertible
- Vision – Eye Med network. $10 deductible exam/$25 materials. $130 frames / $130 contacts
- Accident Coverage – Pays scheduled benefits for accidents from burns, dislocations, and broken bones
- Critical Illness – Flat $5,000 or $10,000 if diagnosed with cancer, heart conditions, or covered illnesses
- Dental – $50 deductible (waived for preventive care) 100% preventive, 80% basic, 50% major – $1000 annual benefit per covered member – enhanced benefits if in-network (providers can be found at, and then follow prompts under “find dental and vision providers near you”)
Platinum Option: (Averages $100 PEPM)
- Life – $10,000 basic group term life insurance. Coverage reduces 50% at age 70 and is convertible
- Vision – Eye Med network. $10 deductible exam/$25 materials. $130 frames / $130 contacts
- Accident Coverage – Pays scheduled benefits for accidents from burns, dislocations, and broken bones
- Critical Illness – Flat $5,000 or $10,000 if diagnosed with cancer, heart conditions, or covered illnesses

- Short-term Disability – Pays 60% of weekly covered earnings to $1000 maximum for 12 weeks after satisfying a 7 day elimination period
- Long-term Disability – Pays 60% of monthly covered earnings to $6,000 after satisfying a 90 day elimination period (seamless coverage from short term to long term disability), and pays a monthly benefit to social security normal retirement age
